YOUR FUNDING

Support at Home,
explained plainly.

Support at Home is the Australian Government program that funds help for older people to live safely and independently at home.

Here's how you apply, what you'd get, and what it costs you.

Four steps to Support at Home funding

GETTING FUNDED

Note: The whole process runs through My Aged Care.

It takes time, and there's a waitlist — so the sooner you start, the better. Call us if you'd like a hand with any of it.

Apply for an assessment with My Aged Care, online or over the phone. You can also ask a GP, health professional or hospital to refer you.

  • 1800 200 422

If you're eligible, you'll be booked in for an assessment. You generally qualify from 65, or from 50 for Aboriginal and Torres Strait Islander peoples.

1 Check your eligibility

Assessments now run through the Single Assessment System — one nationwide process that replaced the older pathways under the Aged Care Act 2024.

An assessor will meet you at home or speak with you over the phone, and work out the level of care you need and how urgently. Bring family or anyone you trust to the assessment.

2 Have your aged care assessment

My Aged Care posts you a notice of decision and a support plan. Log into your My Aged Care online account to:

  • See which services you've been approved for

  • Get your referral codes

  • Review the support plan your assessor wrote

You're then placed in the Support at Home Priority System until funding is assigned. It's a waitlist ordered by need and circumstance, not by where you live.

3‍ ‍Get your assessment outcome

Once funding is assigned, your referral code activates it.

Give us a call with that code and we'll get your supports started.
